Waiver of Liability and Assumption of Risk

WAIVER OF LIABILITY AND ASSUMPTION OF RISK READ CAREFULLY BEFORE SIGNING I understand that participation in Adult Basketball League involves physical activity and inherent risks of serious injury or death. ASSUMPTION OF RISK I acknowledge that basketball involves: • Physical contact between players • Risk of collision with other players, the court, or equipment • Risk of sprains, fractures, strains, concussions, and other serious injuries • Risk of cardiac events or other medical emergencies • Risk of permanent disability or death I voluntarily choose to participate in these activities with full knowledge and acceptance of these risks. RELEASE OF LIABILITY In consideration of being allowed to participate, I, for myself, my heirs, and any legal representatives, do hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE the league organizer, the facility, facility owners, employees, agents, and other participants from any and all liability for: • Personal injury, accidents, or illnesses (including death) • Property damage or loss • Any claims arising from my participation in league games and activities Exception: This waiver does not release anyone from liability for intentional, willful, or wanton misconduct. MEDICAL AUTHORIZATION I authorize the league organizer to seek emergency medical treatment for me if necessary and I am unable to give consent. I understand this is not a guarantee of treatment but permission to act in my best interest in an emergency. ACKNOWLEDGMENT • I am physically capable of participating in basketball • I have disclosed any relevant medical conditions or injuries • I understand the risks and accept full responsibility for my participation • I have read this entire waiver and understand what I am signing • No one has guaranteed me safe participation or specific outcomes
